Elite DNA offers Targeted Case Management Services in the Southwest Florida area. We are one of the few behavioral and mental health service providers that offer Targeted Case Management, which is a program that allows our Medicaid clients to receive additional support. Targeted Case Managers conduct regular visits with clients and families, complete assessments to identify case management needs, complete service plans and obtain insurance authorizations, and overall, manage an active caseload.
Targeted Case Managers are required to have a bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, or related human services field, or have at least one year of full-time equivalent experience working with adults and/or children with serious emotional issues. Once hired, you will be required to obtain a Certified Behavioral Health Case Manager (CBHCM) provisional certification within 3 months.
